Vaccine Exemptions
Healthcare-related academic programs may have a health and safety policy that requires students to receive certain vaccinations. Reasonable accommodations may be available for students with documented disabilities and/or sincerely held religious beliefs that prevent them from getting vaccines.
It is important to understand that although WCTC may grant an exemption, clinical sites ultimately set their own requirements. As a result, some clinical sites may require vaccinations without offering exemptions, or may not recognize or accept exemptions granted by WCTC and this may impact your ability to complete your program.

Religious Exemption Request
Students seeking accommodation due to a sincerely held religious belief may complete one of the following forms:
- Religious Request for students ages 17 and under
- Religious Request for students ages 18+
- Please complete the WCTC Medical Exemption Form and submit with your request.
Medical Exemption Request
Students seeking accommodation due to documented disability may complete one of the following forms:
- Medical Exemption for students ages 17 and under
- Medical Exemption for students ages 18+
- Please complete the WCTC Medical Exemption Form and submit with your request.
COVID-19/Hepatitis B
Students seeking to decline the COVID-19 and/or Hepatitis B vaccination may complete one of the following forms:
